CANCELLATION NOTICE
Thank you for deciding to book your surgery with Dr. Anh Lee. As a courtesy to our office and for other patients, please be aware of the cancellation policy. When booking we are securing this date for your surgery, you are not only paying for Dr. Lee’s expertise but also for operating staff personnel, anesthesia and other resources, costs are incurred regardless of whether the surgery proceeds or not.
CANCELLATION POLICY:
- 50% OF THE TOTAL PROCEDURE FEE WILL BE RETAINED IF SURGERY IS CANCELLED 3 WEEKS PRIOR TO SURGERY FOR ANY REASON.
This fee is charged due to securing the date held specifically for you!
(Example: DR. LEES TOTAL FEE: $10,000.00 – $5,000.00 will be retained)
- FACILITY & ANESTHESIA FEE WILL BE RECOLLECTED IF RESCHEDULED WITHIN 2 WEEKS OF SURGERY**
- TOTAL FEES WILL BE RETAINED IF CANCELLED/RESCHEDULED WITHIN 3 WEEKS FOR ANY REASON*
- FAILURE TO COMPLETE MEDICAL CLEARANCE TESTING, PREGNANCY, OR FAILURE OF NICOTINE TEST ARE NOT MEDICAL REASONS FOR CANCELLATION.
- 100% NON-REFUNDABLE AND NON-TRANSFERABLE
- RESCHEDULING TO ANOTHER DATE – No Fee the first time. The second time and thereafter $1000 fee for each reschedule date.
- PAYMENT PLAN: 50% OF THE PAYMENT PLAN DEPOSIT WILL BE RETAINED IF THE PAYMENT PLAN IS CANCELLED WITHIN 6 months THE FULL AMOUNT PAID TO THE PAYMENT PLAN IS RETAINED AFTER 6 months.
(SURGERY MUST BE DONE WITHIN A YEAR)
I have read and understand the above information regarding my surgery. I acknowledge the procedure listed, and the facility fees. I understand that if I choose the Payment Plan option, my surgery will be scheduled when the final payment is made.
**Every attempt will be made to answer all questions and alleviate any concerns regarding your surgery. However, if circumstances arise and the concerns result in cancellation of your surgery, by you or by Dr. Lee, cancellation fees will still apply.**
Please be advised that we have these policies in place for our Practice and this is to ensure that we can secure a date for you and that our time is valued.
